For GCash Card and AMEX transactions, you must download this form and submit it via a support ticket.

How to Access the GCash Dispute Form

To obtain the dispute form:

  1. Go to the GCash Help Center at help.gcash.com
  2. Navigate to GCash Card > Dispute a Transaction
  3. Download the Subscriber Dispute Form PDF
  4. Submit via ticket: select Pay Online > Your concern > GCash Card or AMEX

How to Fill Out the GCash Dispute Form

Section 1: Personal Information

  • GCash Card Number: Enter your 16-digit GCash card number
  • GCash Registered Mobile Number: Your registered Philippine mobile number
  • Full Name: As registered in your GCash account

Section 2: Transaction Details

  • Transaction Date: The exact date of the disputed transaction
  • Merchant Name: Exactly as it appears in your transaction statement
  • Transaction Amount: The amount that was charged
  • Transaction Reference Number: Found in your transaction history

Section 3: Reason for Dispute

Select the most accurate reason for your dispute:

  • Duplicate Charging: You were charged more than once for the same transaction. Attach a copy of the receipt you authorized.
  • Unauthorized Transaction: The transaction was not made or authorized by you. State that the card was in your possession at all times. Attach front and back of card, and receipt if available.
  • Paid by Other Means: You already paid via another method. Attach proof of the alternative payment.
  • Cancelled Membership/Subscription: You cancelled your subscription. Cancellation must have been done 15 days prior to submission. Attach cancellation confirmation.
  • Merchandise Defective or Not as Described: Goods received were defective or different from what was advertised. Attach documentation and describe the discrepancy.
  • Goods/Services Not Received: You paid but never received what you ordered.

After Submitting the Form
